Aging Waiver Service Coordination


Through Aging Waiver Service Coordination, trained service coordinators work with seniors and their families to develop and implement an individual service plan to help maintain the senior in their own home. 

An experienced service coordinator can help identify the home and community-based services best designed to meet the needs of older adults.

Some of the services available include:

  • Aides to help with activities of daily living (bathing, dressing), as well as help with meal preparation, laundry, and grocery shopping

  • Durable medical equipment

  • Home safety evaluations

  • Home-delivered meals

  • Home health services, such as nurses, physical therapists and occupational therapists

  • Non-medical transportation

  • Respite care

  • Ask your service coordinator if you have a specific need or request!

Program Qualifications

  • Age 60 and older 

  • Living in Montgomery County or the neighboring areas of Bucks, Chester, Delaware, and Philadelphia counties
