Aging Waiver Service Coordination

Through Aging Waiver Service Coordination, trained service coordinators work with seniors and their families to develop and implement an individual service plan to help maintain the senior in their own home. 

Through Aging Waiver Service Coordination, trained service coordinators work with seniors and their families to develop and implement an individual service plan to help maintain the senior in their own home. 

An experienced service coordinator can help identify the home and community-based services best designed to meet the needs of older adults.

Some of the services available include:

  • Aides to help with activities of daily living (bathing, dressing), as well as help with meal preparation, laundry, and grocery shopping

EAGLEVILLE, PA, February 28, 2017 — Family Services of Montgomery County announced today that it has named Timothy F. O’Connell as executive director. O’Connell has served as director of finance of Family Services for 20 years.

On behalf of the staff, volunteers and clients of Family Services’ Meals on Wheels program, I would like to thank the greater Pottstown community for their support of the 5th Annual Meals on Wheels Fundraising Event featuring The Fabulous Greaseband on Saturday, October 1. We welcomed more than 410 energetic music fans to SunnyBrook Ballroom that night, including several birthday and wedding anniversary celebrations.
